Developed by Ontario’s 14 Community Care Access Centres, Bringing Value Home identifies 5 actions that must be taken to ensure our health care system can meet access and quality of care needs of CCAC clients in today’s challenging economic environment.

Community Care Access Centres are:

  • Comprised of 14 local CCAC organizations across the province to manage local care.
  • Funded and legislated by the Ontario Ministry of Health and Long-Term Care.
  • Connectors to home care, long-term care destinations, and other services in your community.

CCAC Quality Report:

CCACs tell the story of our strategy to continuously improve quality of care for Ontarians, including:
  • Showing how we measure our efforts to deliver quality care;
  • Sharing information about who we are and what we do;
  • Presenting innovative programs, partnerships and examples of system integration being delivered to provide higher quality, higher value to our healthcare system.
