Accessing information on health and social services, is quick and easy now with SouthEasthealthline.ca .  On December 3rd, the South East CCAC migrated from communitycareinfo.ca to SouthEasthealthline.ca, putting reliable information about health resources in your communities at your fingertips. The South East CCAC maintains the web-based information database, containing over 6000 records on health and social services which is available to you at:

Looking for a Family Physician or other Primary Care provider?

The CCAC has nurses on staff, called Care Connectors, who work with local physicians and other providers to help link people in need of a primary care provider.To register with the Care Connector program, call 1-800-445-1822 or
