Information and Referral

The South East CCAC maintains an up-to-date information database containing over 6000 records of health and community support services available at:

www.communitycareinfo.ca

Our dedicated Access Team is also available to help with information or service request, from 8 a.m. to 8 p.m. seven days per week by calling:

Ontario's CCACs also collaborate to share information about health and community support services, so it is easy to find information about health and community services – for yourself or for a loved one - anywhere in Ontario.
