SECCAC Suportive Care Team

The Supportive Care Team provides continuity of care and co-ordinated community services to supportive care clients and their families.  Physical, psychological, spiritual and information needs are addressed and optimal well being is promoted along the continuum of illness.  The team is a resource for CCAC colleagues, hospital and community partners regarding supportive and end-of-life care for all clients.
