What is ICCP?

The Integrated client care Project (ICCP) ia a multi-year initiative creating integration and value in Ontario’s health system. It develops, implements, evaluates new and existing home care model delivery while focussing on improving value and quality for clients and the healthcare system.
ICCP organizes home care client service delivery around the client’s clinical conditions (specific client populations), focusing on:

Coordination
Ensuring the right combination of service providers, and enhancing team performance through the management and timing of good client care

Integration
Integrating services through the development of multi-disciplinary care teams (professional and other); and

Specialization
Organizing care around clinical circumstance/client care groupings and focusing care to achieve higher quality and better value
