Strategic Plan 2011-2013

This Strategic Plan represents a roadmap for the Champlain Community Care Access Centre (CCAC) over the next 3 years. It identifies key opportunities and drivers for home and community-based health care in the Champlain region and sets priorities for the organization to follow.
 
The three strategic directions – Service Delivery Excellence; Leadership in Home and Community Care in an Integrated Health System; and Organizational Capacity and Performance – were identified during a thorough consultation process with staff, partners and other key stakeholders.
