The Champlain CCAC helps people who are recovering from a hospital stay needing specific care, have a chronic illness, are living with disabilities, or face other health challenges.

To find out whether you're eligible for our services, contact the Champlain CCAC.  Or you may be referred to the CCAC by a doctor, social worker, family members, friends or others who are concerned about your wellbeing.
