Our Services and Programs

  • Have you recently been released from hospital after surgery or a serious illness?
  • Does your neighbour need a little extra help to cope at home?
  • Does your child need support of a health professional at home or in school?
  • Is your father no longer safe living alone and ready to move into a long term care facility?

These are just a few of the reasons you might turn to a Community Care Access Centre (CCAC).

The Community Care Access Centres (CCACs) are the local point of access to community-based health care services. CCACs are funded by the Ministry of Health and Long-Term Care and were created to coordinate a variety of health services to maintain an individual’s health, independence and quality of life.

CCAC staff can assist you in planning and making informed choices about health care options. We work in partnership with clients, families, care providers, hospitals, long term care centres and others to ensure that the right care is available at the right time and place.

Referrals to Service

Anyone can make a referral to a CCAC — you, a family member, caregiver, friend, physician or other health care professional. To make a referral, for more information or to request a referral form please contact us.

CCAC Case Manager or Care Co-ordinator

When you contact a CCAC you will speak with a case manager or care coordinator. They are regulated health professionals with specialized knowledge and skills that will:

  • Assess your needs by incorporating input from you, your health care professionals and your family and/or caregiver
  • Collaborate with you on setting goals, creating timelines, and carrying out your health care plan
  • Determine your eligibility for government-funded services and settings
  • Provide information and link you to many home and community support services and residential care in your local area and throughout Ontario
  • Act as a link between you and CCAC contracted health care providers
  • Arrange for delivery of government-funded home and community support services
  • Revise your health care plan as your goals and condition change
  • Develop a discharge plan when your goals have been achieved
  • Provide information and referrals to the public about other community agencies and services
  • Provide information about and referrals to other community services

Types of health care services provided through a CCAC

  1. In-Home Services

    These are services that are provided to you or someone you care about within their own home. A case manager or care coordinator will conduct a health care assessment and develop a service plan that meets individual needs. Services are coordinated and monitored regularly to reflect your changing needs. All CCACs contract with service provider agencies to provide in-home services. If eligible, the health care services you may receive include:
    • Nursing
    • Personal support (help with bathing, dressing, etc.)
    • Physiotherapy
    • Occupational therapy
    • Speech language therapy
    • Social work
    • Nutritional counseling
    • Medical supplies and equipment

    Our Service Providers Under the Long Term Care Act, 1994, the CCACs purchase the highest quality and best-priced services from local for-profit and not-for-profit service providers through a request-for-proposal (RFP) process.

  2. Specialized programs

    We also offer specialized programs geared to specific health needs:
    • Acquired brain injury
    • Convalescent Care
    • Child and family services
    • School Health Support
    • Mental Health
    • Palliative care (care at the end of life)
    • Respite care (care that provides a break for the caregiver)

  3. Information and Referral

    If you are not eligible for our services, or if we are unable to provide services to meet all your needs, we will help link you to alternate services available in your community. These may include:
    • Adult day programs
    • Meal delivery services
    • Assistance with shopping or cleaning
    • Transportation assistance

    If you obtain these services directly through providers there may be a consumer fee.

  4. Placement into Long Term Care Homes

    When living independently is no longer possible we coordinate applications to Long Term Care Homes in the area and across Ontario. A CCAC Case Manager will:

    • Provide information about long term care homes
    • Will determine suitability and eligibility for placement
    • Provide assistance in the application process
