Care Coordination
Care Coordination is the core service offered by Community Care Access Centres in the province of Ontario. As the focal point by which clients access a range of services, Care Coordinators collaborate with clients, families, caregivers or substitute decision makers to assist clients in achieving their short and long-term health care goals.
Care Coordinators are committed to:
- Understanding each client's unique needs, circumstances, values and preferences;
- Supporting clients in their efforts to achieve optimal health and independence;
- Advocating on behalf of clients, identifying service gaps, seeking solutions and ensuring each client's right to access and receive quality information and care;
- Assessing and evaluating client needs on an ongoing basis, and ensuring that services are appropriate in relation to their current needs;
- Optimizing each client's ability to achieve their goals, and measuring the effectiveness of service plans;
- Promoting service quality and ensuring the appropriate use of resources
Care Coordinators act as the vital link between their clients and various health care practitioners. This unique relationship enhances the development of the client's sense of empowerment, and ability to live independently and make their own health choices.
Care Coordinators are health professionals and social service professionals and require a B.Sc.N (or diploma), B.Sc.PT, B.Sc.OT, BSW or MSW combined with at least four years of community health or related clinical experience providing client-focused care. Licensure or registration according to the requirements of the profession in the Province of Ontario (or equivalent) as well as a car and valid driver’s licence are requirements of the position. In addition, Care Coordinators must have strong computer and decision-making skills, and the ability to work as part of an interdisciplinary team.

Learning and Development Philosophy
As a learning organization, the Champlain CCAC is committed to lifelong learning and continuous professional development. We partner with employees to provide access to a range of learning and development opportunities, beginning with an initial 4 week orientation and mentorship program and continuing throughout the employee’s career with the CCAC. Employees are encouraged to participate in various in-house education sessions, local training events, and national conferences.

Community Profiles
The Champlain region includes Eastern Counties, Renfrew County, and a portion of Lanark, Leeds & Grenville, spanning a diverse demographic population of both rural and urban clients. With branch and satellite offices spread throughout the region, employment with the Champlain CCAC offers staff and their families a variety of choices with respect to community and lifestyle.
Only an hour away from both the National Capital and Montreal, Cornwall residents enjoy a charming, historic community of 45,965 nestled on the banks of the beautiful St. Lawrence River.
With over 170km of recreational pathways, parks and forests, historic sites and national museums, Ottawa is an urban centre with a small town feel. Some 90 kilometres of scenic parkways run through the Capital, following the waterways from east to west and penetrating to the north into Gatineau Park and the Greenbelt into the south.
Just 40 minutes from Ottawa is our brand new office location in Carleton Place. Beautiful streetscapes, scenic trails and picturesque waterfront make this town one of a kind in the Ottawa Valley. The conveniences of the city, with the friendliness of a small town make Carleton Place a wonderful place to live.
The seat of Renfrew County, Pembroke is situated where the Ottawa River meets the Muskrat River. Pembroke plays a key role as the largest commercial service centre between Ottawa and North Bay, drawing from a regional population of over 100,000.
