ICC Project Interim Contract Templates

 
These documents were developed through consultation with both CCAC and Primary Service Providers. The templates are being utilized to drive discussion and provide clarity to both CCACs and providers in the early implementation sites of the Integrated Client Care Project. The Project supports creativity and innovation in the exploration of methods to increase value in the home care system; however, this understanding is coupled with the need for Standard General Condition changes and flexible local agreements that provide clarity to the participant sites. It is intended that once the outcomes of service delivery and the new relationship are studied, a common template will be achieved.
 
In summary:
1.       The Form of Agreement is completed by the CCAC and includes language specific to the project – process for changes to the services schedule, agreement on intellectual property, start and end dates of the contract.
2.       The Instructions assist the CCAC in completing the documents.
3.       The Service Schedule has the current language for nursing, personal support, physiotherapy, occupational therapy, dietetics and social work combined into one document.
4.       The Service Schedule Adjustment form is used to change the Service Schedule. Some sites may do this immediately upon signing.
 
In addition to these documents, the CCACs also have amending templates for the volume adjustments required to free up volume for the wound care contracts.
