An important part in a person’s care journey is the connection, facilitated by CCACs, between the person seeking care and their local service providers who deliver in-home and community care. Each of the 14 local CCACs in the province of Ontario is responsible for developing strong partnerships with local service providers to realize our vision of providing outstanding care to every person, every day.
We select our partners through an open and transparent Request for Proposal (RFP) process. Service contracts are being awarded to not-for-profit and for-profit provider organizations who exhibit dedication to the high standards required of them. Through this process, service providers are able to demonstrate to CCACs that:
- We are partners in a shared commitment to providing outstanding care to those we collectively serve
- The accountability is shared in CCACs being recognized as a “trusted quality care connector.”

Community Partners
Along with the work that we do with service providers, we also work with a multitude of agencies in your community to provide connections that will meet your health-care needs. You will find CCAC representatives working in your local hospitals to assist with your move back into your home or an appropriate care option; working with a variety of local community support agencies to enrich your day-to-day living and maintain your independence; collaborating with physicians and nurses to ensure you are receiving the care you need; as well as partnering with local educational institutions, from universities to grade schools to support research initiatives and family care.
