Geriatric Resource Team

As we get older, many things change. What remains the same, however, is the desire to live life to one’s maximum potential.

Who is the Geriatric Resource Team?

The Geriatric Resource Team is made up of specially-trained health care professionals skilled at assessing the whole person. A Geriatric Case Manager will interview you and your family in your home to help us understand the unique physical, mental, emotional and social challenges you are facing. The Geriatric Resource Team will strive to improve your situation.

How can the Geriatric Resource Team help?

The goal of the team is to foster independence, reduce risks and help elderly people live richer, more satisfying, lives.
 
Following an in-depth assessment, the Case Manager will have several intervention options available to assist you. These may include:

  • Providing education to you and your family about the health care challenges you are facing
  • Recommending linkages to other community agencies for support
  • Helping you navigate the many regional programs available to enhance your current situation
  • Accessing the expertise of physicians who specialize in the care of the elderly to review your case and liaise with your family doctor for ongoing medical support as required

Who should have an assessment?

  • People over the age of 65 with an age-related health problem
  • People showing signs of memory loss or behaviour problems
  • People prone to falling
  • People having difficulty managing on their own
