Sandra Coleman
Chief Executive Officer
Sandra Coleman has been the CEO of the South West Community Care Access Centre since its inception in January, 2007. Previously, Sandra was Executive Director of the Community Care Access Centre London Middlesex since February, 2002, and a member of its senior leadership team since 2001. The South West community care team has worked in close partnership with its service provider, hospital, long term care home and community support service agency partners to provide high quality home care, implementing many innovative new programs to support clients remaining in their homes.
Sandra is currently Vice Chair of the Ontario Association of Community Care Access Centres (OACCAC), and a past member of the Board of Directors of the Ontario Hospital Association and of the Board of Directors of the Thames Valley Family Health Team. Sandra has also participated in and led numerous provincial initiatives.
Sandra previously practiced law with Lerner and Associates in the areas of health care, human rights, employment and appellate law. She has litigated in the Supreme Court of Canada, the Ontario Court of Appeal and the lower courts.
Sandra has a Bachelor of Laws from the University of Western Ontario where she was the recipient of the Gold Medal, and has an Honours Bachelor of Business Administration from Wilfrid Laurier University.
Sandra has been actively involved as a volunteer with numerous organizations, including Board Chair of Hospice of London for five years during which time she was a leader of their capital campaign which culminated in their new Home for Hospice. She has been awarded the Lieutenant Governor’s Community Service Award and the Province of Ontario’s Leading Women Building Communities Award.
Sandra resides in Middlesex County, near London, Ontario.

Donna Ladouceur
Senior Director, Client Services
Donna Ladouceur has been in role as the Senior Director, Client Services for the South West CCAC since its inception in January 2007. Previously Donna was the Director of Client Services since 2005. Donna has worked in the community since 1990 providing case management for hospitals, community and the children services team as well as leading the development of the End of Life Strategy for London and Middlesex. She was a branch manager prior to that for a community healthcare agency.
Donna has 10 years of senior leadership experience in a Hospital based research clinic which was a joint venture between an acute care hospital and a pharmaceutical company, the first such venture in Canada.
Donna has a strong, demonstrated track record in leading the delivery of client-driven care which is innovative, integrated and accountable. She is a passionate advocate for consistency and equitable services delivery to clients and has a proven track record as a change leader and proven ability to create and sustain strong strategic partnerships. Donna resides with her family in Middlesex County.

Nancy Dool-Kontio
Senior Director, Strategic Planning and Integration
Nancy has been in management positions with the public and private sector for 14 years and has been a Manager with both the CCAC of London-Middlesex and Elgin CCAC. During her tenure with CCACs she has played an instrumental leadership role in creating innovative proposals in the development and implementation of system initiatives that support the CCAC in provision of enhanced services to clients and families. These include Children’s Services, Peritoneal Dialysis, Early Ortho and Prehab programs, and the Thames Valley Family Health Team. She has proven ability to build strategic system partnerships to generate solutions that meet evolving health care needs and has demonstrated her commitment to health system integration by using her research and development skills to create innovative proposals. |

Gordon Milak
Senior Director, Performance Management and Accountability
Gordon has over 20 years of experience in operational management and has held senior positions with Victorian Order of Nurses including Executive Director of both VON Middlesex-Elgin and VON Chatham-Kent, as well as Director of Business Development for VON Canada-Ontario. He has a proven track record of building strong relationships and partnerships with stakeholders and is a champion of change and transitional management. His community experience includes membership on the Salvation Army Community Advisory Committee, Family Health Team Steering Committees, and End of Life Planning Committee. |

Hilary Anderson-Halliday
Senior Director, Corporate Services
Hilary Anderson-Halliday has been in management positions in the public sector for 20 years and has been a Manager with the London-Middlesex and now South West CCAC for more than 10 years. While with the CCAC, Hilary has demonstrated leadership in the areas of finance, reporting, control management in addition to workflow processes and measurement.
No one is more passionate about our clients and the stewardship of the taxpayers' dollars than Hilary.
Hilary resides with her family in London.

Jacqueline Mockler
Senior Director, HROD
Jacquie has been a leader within the HROD team since 2008 and possesses a Bachelor of Arts from the University of Western Ontario, Certificate in Human Resources Management from the University of Toronto and is a Certified Human Resources Professional with significant HROD experience.
As Regional Manager, Human Resources for the South West CCAC, Jacquie has participated in a number of strategic initiatives inclusive of leading the labour relations portfolio and co-leading the implementation of the Population Based Approach to Client Driven Care demonstrating a commitment to transformative leadership. Throughout Jacquie’s time with the South West CCAC she has partnered in creating a great place to work and actively collaborates and engages with employees, teams and union partners in developing a shared understanding, building trusting relationships and co-creating ways to enable the delivery of outstanding care. |
