Convalescent Care
The South West CCAC provides support and resources including:
- Information and referral to community resources
- System navigation
- Access to long term care and day programs
- Case management services including individualized assessment and Service Planning for in home services and supports
Discharge from Hospital
You may require assistance when you return home from hospital. We know that it is difficult to know where to start when you are looking for help and you may not realize what services are available in your community.
You can arrange a number of support services directly. If you require help with planning your needs and/or you will require personal care or professional services, a CCAC Case Manager can help.
In general, you should:
- Plan ahead. Most services can be arranged in advance
- Enlist the support of family and friends for help upon discharge;
- Anticipate your need for help with things like laundry and housekeeping. Think about daily activities that may be affected and who you can get to assist
- Think about equipment that you may need (for example, a cane, a walker, etc.)
- Consider using private services (for a fee) for a variety of supports
- Think about delivery services that may be available from your local pharmacy
- If you require assistance with personal care, the CCAC may be able to purchase this service for you. A Case Manager from the South West Community Care Access Centre will assess your needs and eligibility for services; and
Professional services such as nursing and therapies will be arranged for you by the CCAC, when necessary, prior to your discharge from the hospital.
Through agreements with hospitals South West CCAC Case Managers perform the discharge planning function for patients being discharged home with South West CCAC services and admissions to long-term care homes. These arrangements provide enhanced continuity of care for clients, eliminate duplication of function, and provide opportunities to ensure efficient and effective service coordination.
