School Health Support Services

Three Ministries share the responsibility for providing health support services to school aged children: the Ministry of Health and Long-Term Care, the Ministry of Education and the Ministry of Children and Youth Services.

What are the services?

  • Case Management
  • Nursing
  • Occupational Therapy
  • Physiotherapy
  • Nutrition
  • Speech Therapy
  • Personal Support

Who can access the services?

Students who:

  • Attend an elementary or secondary school governed by the Education Act
  • Attend Private or Home School
  • Have a valid Ontario Health Card
  • Need special health services at school which are essential to their participation and development in school programs

How to get help for a student

  • Teacher/Parent identifies a need or concern
  • With parent's/guardian's permission school initiates request for service by contacting South West CCAC Case Manager to consider student's eligibility for School Health Services

What does the South West CCAC Case Manager do?

  • Assesses eligibility for School Health Support Services according to Ontario government legislation
  • Plans and coordinates all school health support services and funding in collaboration with the community care team
  • Links the client/family/school with other appropriate resources

What are the responsibilities of the school?

  • To incorporate the strategies and recommendations developed in collaboration with the community care team into the student's school routine
  • To provide space for service delivery

What are the responsibilities of the parent?

  • To actively participate as a member of the care team
  • To practice the strategies and recommendations that have been developed in collaboration with therapists/nurses
  • To communicate any concerns to service providers, school or South West CCAC Case Manager
  • To notify the service provider agency if the student will be absent from school while receiving professional services so that visits can be rescheduled

How long do the services last?

Services stop when:

  • Professional consultation is complete
  • The student has achieved the goals of the special health services
  • The student is not benefiting from the special health services or progress towards goals has reached a plateau
