Ensuring that seniors have access to the right care, at the right time and in the right place, the Seniors Care Strategy announced in Ontario’s Action Plan for Health Care will look at how to support seniors at home and reduce hospital readmissions and pressure on long-term care homes.

Having a pivotal role in caring for Ontario’s seniors, CCACs have submitted their recommendations to improve seniors’ care based on their collective experience in caring for over 300,000 seniors each year in the Senior’s Care Strategy submission titled Improving Care for Seniors and Caregivers.
