Setting the Future Course for Home and Community Care in Waterloo Wellington

Community Care Access Centres are recognized as care coordinators and system navigators helping to build a seamless and coordinated health care experience for the residents of Waterloo Wellington. This spring, WWCCAC will have a refreshed strategic plan reflecting this evolving role within the local health system. Care coordination will continue to be at the heart of CCAC’s future planning. The WWCCAC has and will continue to engage staff, patients/families, community partners and health system planners to help inform the development of the organization’s future priorities.
