PROJECT STRUCTURE
ICCP was originatlly a partnership established between Ontario's Ministry of Health and Long-Term Care, the Ontario Association of Community Care Access Centres (OACCAC), the Collaborative for Health Sector Strategy, University of Toronto, Rotman School of Management and the Local Health Integration Networks (LHINs).
ICCP-Wound Care included three co-sponsors: the Ministry of Health and Long Term Care (MOHLTC), the Ontario Association of Community Care Access Centres (OACCAC) and the Collaborative for Health Sector Strategy at the Rotman School of Management at the University of Toronto. ICCP-Palliative Care also includes sponsorship from Ontario's LHINs.
