Palliative Care
The CCAC provides specialized services for individuals who are facing a progressive life threatening illness. The palliative care team is committed to providing sensitive and skilled care to meet the physical, emotional and spiritual needs of both yourself and your family. Care Coordinators on the palliative care team have special knowledge of palliative care issues and community resources to assist you.
Your Care Coordinator will work with you to:
- Provide support and resources to you, family, friends and caregivers;
- Provide you and your family with information about community support groups;
- Provide you with pain and symptom management;
- Counsel you on the options of hospital respite or admission into a hospice, including hospice day programs where available;
- Offer education, counselling and emotional support to help you cope with the challenges of the disease process;
- Provide bereavement support and information on community bereavement resources.
Your Care Coordinator will work closely with your physician and service providers as well as others involved in your care.
The CCAC can assist you with hospice admissions. Your Case Manager will continue to collaborate with you, and your family to assist you in making your adjustment as comfortable as possible.
