Children with special health needs are eligible for the program, whether they attend a publicly-funded school, a private school, or are home schooled. A valid Ontario Health card is required.
Parents, teachers or others connected to the care may make a referral to the program; a doctor’s referral is not necessary. Referral forms are available in all schools.
