Community Care Currents

"Community Care Currents" is a newsletter for health system partners and stakeholders of Central West Community Care Access Centre (CCAC). Each issue of "Currents" will focus on timely and relevant topics that highlight Central West CCAC's evolving role in an integrated health system. In this issue, our collaborative efforts in reducing ALC days and improving patient flow, particularly with William Osler Health Centre, our largest hospital partner, are profiled. "Currents" will be produced three to four times annually. We hope you enjoy this new publication and that you find the information useful and informative.
