Eligibility
To qualify to move into a long-term care home, you must:
- Be 18 years of age or older
- Possess a valid Ontario Health Card
- Have health care needs that cannot be met with any combination of informal caregiving or community-based services in the home
- Have health care needs that require, and can be met in, a long-term care home
If you are deemed ineligible and you feel that you are eligible, you have the legal right to appeal the assessment. If you choose to start the appeal process, your CCAC Care Coordinator can give you the appropriate phone numbers and contact information.
How do you apply?
Application for admission into a provincially regulated long-term care homes can only be made through a CCAC. Please contact the Central CCAC to initiate a referral for placement.
CCAC staff will answer your questions and help with the application process. We will assess your health care needs to determine if you are eligible and provide you with information to help make this important decision. We can also assist you in accessing community support services that may help you manage until admission is arranged.
Long-term care home costs
The Ministry of Health and Long-Term Care pays for the care you receive, but you are required to pay for the accommodations in a long-term care home. The amount paid by residents for their accommodation is called a co-payment. These rates are adjusted annually by the Ministry and are the same across Ontario.

Rates effective July 1, 2012
Long-term care homes cannot refuse admission to eligible individuals based solely on inability to pay for accommodation.
If you do not have enough annual income to pay for the basic room, the government can help you through a rate reduction that may lower your monthly costs. The subsidy is not available to people requesting semi-private or private rooms.
If you wish to apply for a rate reduction, the CCAC can provide you with an information package that contains a list of required documents and detailed instructions to complete the appropriate application.
