The Central East Community Care Access Centre is one of six centres across Ontario under the Ministry of Health and Long-Term Care Ontario Diabetes Strategy to establish a Centre for Complex Diabetes Care (CCDC).  The Central East CCDC is a regional care delivery program servicing all of the Central East Local Health Integration Network.

  • Funded and legislated by the Ontario Ministry of Health and Long-Term Care.
