4/30/2013 Rapid Response Nurses help patients manage the transition home
Patients moving from hospital to home will receive additional care to ensure a successful recovery through the local Community Care Access Centre’s Rapid Response Nurse Program.

The South East CCAC has hired five nurses to reduce hospitalization and avoidable emergency department visits by improving the quality of transition from hospital care to home care for medically vulnerable seniors and children.

Families seeking information about long-term care home waitlists can now access a monthly report on the South East CCAC public website to help them make informed decisions.
